Q. What
should I expect on my first visit? |
| |
A. After being greeted by our office staff, you will meet with a Physical Therapist. The therapist will perform a detailed evaluation during your first visit. This will take from 1-1/2 hours. You will begin an initial treatment and be provided with exercises for homework. We will provide you with a welcome packet that includes how to care for yourself after physical therapy.

Q. What
should I bring? |
| |
A. Please bring your license, your health insurance cards and a list of any and all medications you currently take. If you have any allergies we will need to know that as well.
If your physician provided you with a prescription for Physical Therapy Care please bring that as well.
Your prescription will provide us with information to make communicating with your physician about your progress more complete.

Q. What should I
wear? |
| |
A. Loose fitting clothing that allows you to move comfortably. We recommend bringing along or wearing shorts for injuries below the waist. Changing areas are available to you. Comfortable footwear is strongly advised, bringing sneakers is highly recommended.

Q. What kind of insurance
do you take? |
| |
A. We
take most types of insurance including; BCBS New
England, BCBS MA, Anthem BCBS, Cigna, Aetna, United
Healthcare, Harvard-Pilgrim, Liberty Mutual, Tufts,
Sedgewick, Tri-care, Oxford, Orthonet, Medicaid,
Medicare and others.
Our office will file your insurance information
on your behalf and work with you on required referrals.
If you do not see your insurance
provider listed, please call us to confirm as we
are adding providers daily.

Q. I want to pay
my Co Pay...do you take credit or debit cards? Do
you take the Flexible Spending Account card from
my insurance company? |

A. Yes, we do take any type of credit, debit, or Flexible Spending cards from insurance companies. If you know you have a co pay, please bring cash or check. We will provide you with the appropriate receipts to be able to submit to your insurance company.

Q. If I have to
cancel my appointment, will I be charged? |
| |
A. We will not charge you if you need to cancel an appointment. Please give us a call however so that we know in advance.

Q. How do I find out if I have a co- pay, co-insurance, or a deductible for physical therapy? |
| |
A. Call your insurance company. Their number is usually on the back of the insurance card. Tell them you are a patient of Physical Therapy and they will tell you what you will be expected to pay. For your convenience we will also verify your benefits on your behalf.

- Do I have a deductible? Is that per person or per family?
- What is my deductible
amount? Has it been met? How much have I
paid toward my deductible?
- For Physical Therapy
is there a maximum number of visits or maximum
dollar amount that I am allowed per year
(calendar or policy year?)? Per diagnosis?
Per lifetime?
- What is my benefit
coverage and Insurance%? Do I have a co-pay
and is that per visit? How much is that co-pay?
Is that for every type of visit?
- Do I have a maximum
out of pocket limit?
- Do I have co-insurance?
Is that different from my co-pay?
- Do I need to get
a referral or authorization prior to starting
Physical Therapy? Who does that need to come
from?
- Do I have a self
referral benefit? Do I have any limitations
should I self refer?
